超声造影在胃肠肿瘤良恶性鉴别诊断中的初步临床应用 被引量:9

Clinical application of contrast-enhanced ultrasound in differential diagnosis of malignant and benign gastrointestinal neoplasms

摘要 目的探讨超声造影在胃肠肿瘤良恶性鉴别诊断中的应用价值。方法对32例经手术或CT、X线及内镜证实的胃肠道肿瘤患者的超声图像特征和超声造影检查资料进行回顾性分析。结果8例良性病变的造影剂开始增强时间为(37.12±8.21)s,增强持续时间为(22.50±5.55)s,开始消退时间为(59.62±6.94)s;24例恶性病变的造影剂开始增强时间为(15.16±5.35)s,增强持续时间为(16.87±8.61)s,开始消退时间为(32.04±12.79)s。24例恶性病变在造影剂开始增强和开始消退时间上与8例良性病变比较差异有非常显著性(P<0.01)。超声诊断胃肠肿瘤与病理符合29例(29/32),超声造影鉴别胃肠道良恶性肿瘤与病理符合26例(26/32)。结论超声造影检查可为胃肠道肿瘤的良恶性鉴别提供有用的血流动力学信息的影像学依据,具有一定的临床应用价值。 Objective To discuss the value of the differentiation of malignant and benign gastrointestinal neoplasms using contrast-enhanced ultrasound(CEUS).Methods Gastrointestinal neoplasms in 32 cases were analysed using routine color Doppor ultrasound imaging and CEUS, which were confirmed to be malignant or benign through operation/CT/X-rays and endoscope.Results Pathological results of all the cases indicated that twenty-four cases were with malignant and eight cases were with benign. In 8 benign cases the contrast-enhanced agent flow-in beginning time and flow-out beginning time were obviously different from those in 24 cases, ie,37.12±8.21s and 59.62±6.94 s vs 15.16±5.35 s and 32.04±12.79 s, respectively (P<0.01). The accuracy rate of color Doppor ultrasonographic examination was 29/32 and that of CEUS was 26/32. Conclusions CEUS can provide objective and important imaging warranties for diagnosis of malignant and benign gastrointestinal neoplasms which is valuable for clinical application.
