血管回声跟踪技术对高血压伴高脂血症患者颈动脉弹性功能的检测及临床意义 被引量:15

摘要 目的应用血管回声跟踪技术(Echo-Tracking,ET)检测高血压及高脂血症患者的颈动脉弹性功能。方法选择我院就诊的患者40例,其中高血压患者12例,高脂血症患者12例,高血压伴高脂血症患者16例。同期随机选取正常人10例。应用ET评估颈动脉硬化参数,包括压力应变弹性系数(Ep)、僵硬度(β)、顺应性(AC)、脉搏波传导速度(PWVβ)和增大指数(AI),通过这些参数了解高血压伴高脂血症患者动脉弹性的变化。结果高血压伴高脂血症患者Ep、β、PWVβ、AI较正常对照组明显增高,AC明显减小。高血压患者Ep、β、PWVβ、AI较正常人无明显增高,AC无明显减小;高脂血症患者Ep、β、PWVβ较正常人无明显增高,AC无明显减小。结论应用ET能反映血管的早期病变。高血压伴高脂血症患者颈动脉弹性功能发生了改变,因此,对这些患者加强监测可能对于脑血管疾病的防治有一定的价值。 Objective Using echo-tracking(ET) technique to evaluate the carotid elasticity quantitatively in patients with hypertension and hyperlipidemia.Methods Forty patients were enrolled,12 patients with hypertension,12 patients with hyperlipidemia,and 16 patients with hypertension and hyperlipidemia.Another 10 healthy cases were selected as normal control.With ET technique,we calculated the carotid elastic indexes:strain elastic modulus(Ep),stiffness parameter(β),arterial compliance(AC),one-point pulse wave velocity(PWVβ) and argumentation index(AI).Results Compared with the normal cases,the data ofEp,β,PWVβ and AI were significantly higher in both hypertension and hyperlipidemia patients,while AC was obviously lower(P<0.01).However,for either hypertension or hyperlipidemia patients the data of β,Ep,PWVβ,AI and AC had no significant difference from the indexes of normal control.Conclusion ET can evaluate the arterial elasticity of the patients with hypertension and hyperlipidemia.And it may be helpful to prevent cerebrovascular disease.
