宫腔镜电切术治疗异常子宫出血18例临床分析

Clincal Observation of Hysteroscopic Electric Resection:An Analysis of 18 Patients with Abnormal Uterine Bleeding

目的探讨宫腔镜电切术治疗异常子宫出血的临床疗效。方法应用连续灌流式宫腔镜,对18例异常子宫出血患者进行宫腔镜电切手术治疗,其中10例为子宫内膜切除、2例为黏膜下小肌瘤切除、6例为息肉及部分内膜切除。结果术后随访6～12个月者16例,月经改善率为87·5%,其中闭经4·5%,月经量减少68·8%,月经量正常12·5%,3例痛经中2例改善,术后满意率76·5%。术后10个月后月经改善率低于术后6个月,估计与切割深度不足有关。闭经率低可能与刚开展宫腔镜电切手术,切割不完全有关。结论宫腔镜电切术治疗异常子宫出血,短期疗效高、并发症少。长期疗效尚待观察。在提高手术技术和基础知识的同时,应掌握适当的手术指征。

Objective To study the efficacy of hysteroscopic electrosurgery in treating abnormal uterine bleeding(AUB).Methods Eighteen women with AUB received hysteroscopic transcervical resectionof the endometrium or intrauterine lesions with a continuous flow resectoscope.Results Eixteen patients were followed for 6 to 12 months .Menstrual symptoms ameliorated in 87.5% of the pstients.4.5% of the women became amenorrhoeic.68.8% had only spotting,12.5% recoverd normal period.2 patients received hysterectomy later for recurrent uterine bleeding.Conclusion Hysteroscopic electrosurgery is an effective procedure in treating abnormal uterine bleeding.But its long -term efficacy is yet to be identified.When improving the technique and basic knowledge,the indications should be given appropriately.
