Survivin和p53在脑膜瘤中表达及临床意义

摘要 目的探讨凋亡相关基因survivin和p53在脑膜瘤中的表达及其临床意义。方法应用免疫组化SP法,检测97例脑膜瘤组织、18例正常脑膜组织中凋亡相关基因survivin、p53蛋白的表达,结果应用统计学分析。结果Survivin在正常脑膜组织和脑膜瘤组织中阳性表达率分别为0和65·9%(P<0·01)。Survivin表达阳性率在脑膜瘤Ⅱ级(85·7%)和Ⅰ级(51·4%)之间、Ⅲ级(93·8%)和Ⅰ级之间差异具有统计学意义(P<0·01)。p53在正常脑膜组织和脑膜瘤组织表达的阳性率分别为0和27·8%(P<0·05)。p53表达阳性率在脑膜瘤各级别之间的差异具有统计学意义(P<0·01)。Survivin与p53蛋白表达共阳性23例,共阴性29例,呈正相关(rs=0·451,P<0·01)。结论①Survivin和p53在脑膜瘤中的表达与其恶性程度相关;②Survivin基因过度表达和抑癌基因p53失活在脑膜瘤的发生和恶化中起协同作用;③Survivin将是脑膜瘤免疫治疗理想和有效的靶基因。 Objective To study on expression of survivin and p53 in meningiomas and their clinical significance.Methods Ninty-seven specimens of meningioma and 18 normal meninges were detected for expression of survivin and p53 by S-P immunohistochemical method.The results were statistically analysed.ResultsThe positive rates of survivin expression in normal meninges and meningiomas were 0 and 65.9%(P<0.01).Survivin positive expression rates were 51.7%,85.7% and 93.8% in meningiomas of grade I,grade Ⅱ and grade Ⅲ.The positive ratios of survivin showed a significant difference between in grade Ⅱ and in grade I(P<0.01),in grade Ⅲ and in grade I(P<0.01).The positive rates of p53 expression in normal menings and meningiomas were 0 and 27.8%(P<0.05).p53 positive expression rates were 13.3%,38.1% and 68.8% in meningiomas of grade I,grade Ⅱ and grade Ⅲ.There was a significant difference among these three groups(P<0.01).There were 23 patients who had both survivin and p53 expression at the same time,29 patients had negative expression of them.A positive correlation(rs=0.451,P<0.01) between survivin and p53 expression was found.Conclusion ① The expression of survivin and p53 in meingiomas was obviously associated with its malignancy.② The over expression of survivin gene and de-activation of antioncogene p53 play a synergetic role in the process of oncogenesis and malignancy of meingiomas.③Survivin may be an ideal and effective immunotherapeutic target gene in meingiomas.
