肝硬化程度评价:磁共振灌注成像价值研究 被引量:2

摘要 目的探讨磁共振灌注成像图像及量化指标与肝硬化程度间的联系。方法收集48例慢性肝炎患者为实验组,50例肝功能正常者为对照组。均进行常规MRI扫描、动脉期和门静脉期灌注成像,并测量灌注成像量化指标最大增强斜率MSI,比较实验组与对照组肝实质MSI。对实验组根据Child-puge改良分级法进行肝硬化分级。将实验组肝实质MSI与肝硬化分级进行相关性分析。结果动脉期和门静脉期,对照组与实验组肝实质MSI差异有统计学意义(P<0·05)。实验组诊断肝硬化A级6例,B级18例,C级24例。实验组门静脉期肝实质MSI与肝硬化分级间存在相关性。结论磁共振灌注成像可用于诊断肝硬化,其量化指标MSI可反应肝硬化程度,在评价肝硬化中有着重要作用。 Objective To approach the relation of MR perfusion-weighted imaging(PWI)and the degree of liver cirrhosis.Methods 48 patients with chronic hepatitis were collected as experiment group,and 50 normal persons as control group.After undergoing routine MRI and PWI,the quantization index,maximum slop of increase (MSI) was compared between 2 groups.The patients in experiment group were graded with Child-Pugh class.And correlation of MSI and clinic grade was assessed.Results In arterial and portal phase,the MSI was significantly different between patients with and those without cirrhosis.In experiment group,6 cases were with Child-Pugh class A cirrhosis;18 with class B cirrhosis; and 24 with class C cirrhosis.In portal phase,MSI was correlated with Child-Pugh class.Conclusion Hepatic cirrhosis can be detected by PWI.MSI correlate with the severity of cirrhosis.PWI is important to evaluate the degree of hepatic cirrhosis.
