慢性阻塞性肺疾病气道反应性测定的临床价值

Clinical value of determination of bronchial reaction for chronic obstructive pulmonary disease

目的探讨慢性阻塞性肺疾病(慢阻肺,COPD)气道反应性测定的临床应用价值。方法随机测定63例符合诊断标准的COPD患者的气道反应性,并按其结果分为气道高反应(HBR)组33例和气道反应性正常(NBR)组30例,分析两组的临床特点。结果HBR组中慢性喘息性支气管炎(慢喘支)12例,NBR组中慢喘支5例,进行两样本率的χ2检验(P<0·05);HBR组COPD合并哮喘21例,NBR组中COPD合并哮喘6例(P<0·01)。另外,所有研究对象均于第1次肺功能检查后,吸入布地奈德干粉剂,1喷/次,2次/d,持续3个月后再做第2次肺功能检测,对比研究对象用药前后2次的FEV1、FEV1%、MMEF、MVV,HBR组肺功能下降明显延缓。结论部分COPD患者存在气道高反应性,对此类患者吸入激素治疗,有延缓肺功能下降的作用。

Objective To ivestigate the clinical value of determination of bronchial reaction for chronic obstructive pulmonary disease(COPD).Methods The bronchial reaction of 63 cases conform to the diagnosis of COPD were determined,and according to the results they were divided into 2 groups:the high bronchial reaction(HBR) group of 33 cases and the normal bronchial reaction(NBR) group of 30 cases.The clinical characterization of the 2 groups were analyzed.Results In the HBR group the chronic asthmatic bronchitis constituted 12 cases and the NBR group constituted 5 cases,compared with 2 groups' s datum there were significant difference(P<0.05).In the HBR group COPD complicated with asthma constituted 21 cases and the NBR group constituted 6 cases,there were significant difference(P<0.01).In addiction,all the case drawn budesonide after the first check of pulmonary function,one sprayer one time,twice per day and last three months,after 3 months,therapy and then have the second check of pulmonary function,compared the cases' FEV1,FEV1%,MEEF,MVV before and after the therapy,the HBR group patients' pulmonary function decline was obviously been delayed,it had significant difference.Conclusion Part cases of the COPD patients have HBR,if the therapy of drawing glucocorticoids was given,the pulmonary function would be delayed to decline.
