摘要
目的探讨采用微创术式对75岁以上高龄髋部骨折行关节置换的可行性及治疗效果。方法本组56例髋部骨折中,股骨颈囊内移位型39例,股骨转子间骨折17例,均经髋后外侧微创入路,行人工股骨头置换。结果随访6个月至4年。手术全部获得成功,除3例发生深静脉栓塞,无一例大出血及坐骨神经损伤,术后1~2周均可下地行走。无关节脱位。结论高龄髋部骨折患者多同时合并心脑血管或呼吸系统疾病,体质较弱,采用小切口行髋关节置换,可减少创伤、减少出血,大大减少或减轻这些并发症的发生。
Objective To investigate the feasibility and effect of minimally invasive arthroplasty in treating hip fractures for the advanced aged patients of more than 75 years old.Methods 56 cases of hip fracture,aged between 75 and 98 years old,were included for analysis.Among them,there were 39 cases of displaced femoral neck fractures and 17 cases of intertrochanteric fractures.Bipolar hemiarthroplasty was adopted to all the cases through minimally invasive posteriolateral hip approach.Results All the cases were followed-up for a period of 6 months to 4 years.All the operations achieved success.Deep vein thrombosis occurred in 3 cases.Neither massive hemorrhage nor sciatic nerve injury were found.All the patients could get up and move about with weight in 3~4 weeks’time after operations.Joint dislocation was not found after the operation.Conclusion Hip fracture patients of the advanced age mostly complicated with cerebro-cardiovascular diseases and respiratory diseases.They mostly have weak constitutions.Hip arthroplasty with minimally invasive incision is suitable for them,which could diminish trauma and bleeding,and greatly reduce and alleviate complications.
