5-Fu+CF双周疗法联合腹腔化疗治疗晚期消化道肿瘤

目的探讨5-氟脲嘧啶(5-Fu)+醛氢叶酸(CF)双周疗法联合腹腔灌注化疗治疗晚期消化道恶性肿瘤的疗效及不良反应。方法随机分2组,联合腹腔化疗组:采用锁骨下静脉穿刺留置中心静脉导管48小时持续灌注高浓度5-Fu3g/m2,灌注前静滴CF200mg/m2,采取单针腹腔穿刺腹腔化疗:顺铂(DDP)80mg/m2,每2周1周期,对照组:5-Fu500mg/m2·d、CF200mg/m2·d、DDP20mg/m2·d静滴d1-d5,每3周1周期,均化疗4～6周期。结果联合腹腔化疗组:CR1例,PR15例,总有效率61.5%。对照组:无CR病例,PR9例,总有效率33.3%。2组有效率比较,P<0.05,双周疗法联合腹腔化疗组较静脉化疗组有效。联合腹腔化疗组中位生存期15.6月,1年生存率为42.3%,对照组中位生存期11.2月,1年生存率25.9%,两组1年生存率比较,P>0.05,相差无统计学意义。两组骨髓毒性相似,而联合腹腔化疗组静脉炎、消化道反应明显减轻。结论本组以高浓度5-Fu持续滴注+CF生化调节联合腹腔化疗较对照组毒副反应轻,有效率高,可望使晚期消化道肿瘤患者的化疗适应范围增加,使更多病人受益。

Objective To approach carative effect and untoward reaction of the 5-Fu+Cf two weeks treatment the association venter imus filling chemotherapy to treat the advanced stage tractus alimentarius malignant tumor.Methods Random divide into two group, combination venter imus chemotherapy group: they were used subclavian vein puncture to detain central venous catheter 48 hours to last filling the high concentration of 5-Fuused 3g/m2, before filling use intervenous drop infusion CF used 200mg/m2,adopt single needle venter imus puncture venter imus chemotherapy DDP used 80mg/m2, every two weeks for one cycle,control group:5-Fu used 500mg/m2·d、CF used 200mg/m2·d、DDF used 20mg/m2·d、intervenous drop infusion d1-d5,every three weeks for one cycle, they are all chemotherapy 4-6 cycle.Results The association venter imus chemotherapy group: CR have lcase, PR have 15case, total effective rate is 61.5%。Control group: not to have CR case, PR have 9case, total effective rate is 33.3%. Two group effective power to contrast, p<0.05,two weeks treatment association venter imus chemotherapy group have utility contrast vein chemotherapy group. The association venter imus chemotherapy group meso life span for 15.6 months, one year survival rate to contras, P>0.05, phase contrast not to have statistical significance. Two group marrow toxicity resemble, but association venter imus chemotherapy group phlebitis、tractus alimentarius reaction have to lighten obviously.Conclusion This group use high concentration of 5-Fu to last drip+CF biochemistry to regulate association venter imus chemotherapy group have poison side reaction lightly contrast control group, effective power higher, this treatment will to make the advanced stage tractus alimentarius tumor patient's chemotherapy subject range to increase, to make many patient have to profit from.
