48例鼻腔、鼻窦内翻性乳头状瘤鼻内镜下切除的疗效观察

目的探讨鼻内镜下切除鼻腔、鼻窦内翻性乳头状瘤的可行性。方法回顾性分析48例鼻腔、鼻窦内翻性乳头状瘤经鼻内镜下切除患者的术前病情评估、治疗和术后随访资料。结果10例Ⅰ期和28例Ⅱ期鼻腔、鼻窦内翻性乳头状瘤患者行经鼻内镜鼻腔、鼻窦内翻性乳头状瘤切除术,5例Ⅱ期和5例Ⅲ期患者行鼻内镜加Caldwell-Luc术。手术效果良好,随访3～8年,共有3例复发,无恶变患者。结论术前行鼻内镜、CT、MRI检查,可以确定病变范围,根据病变范围决定手术方式,经鼻内镜可以彻底切除Ⅰ、Ⅱ、Ⅲ期病变;Ⅳ期病变仍需传统鼻侧切开。

Objective To study the feasibility of endoscopic surgery for nasal inverted papilloma. Methods 48 cases who were diagnosed as nasal inverted papilloma were analyzed about the estimation before surgery,during the treatment and the follow-up after surgery.Results 10 cases inⅠ period and 28 inⅡperod were operated about nasal inverted papilloma by endoscopic surgery. And 5 cases inⅡand Ⅲ periods by endoscopic surgery with Caldwell-luc. The surgery effect was good; the follow-up was 3~8 years. In all, 3 cases were recurrent but not worse. Conclusions The examination of endoscope, CT and MRI before surgery can determine the disease field, according to which the surgery way can de decided. Endoscopic surgery can excise the disease in Ⅰ, Ⅱ and Ⅲ periods, but the disease in Ⅳ period still need the traditional nasal side excision.
