糖尿病肾病综合征患者应用大剂量转换酶抑制剂及受体拮抗剂治疗研究

The effects of large-dose ACE inhibtor and AⅡ recaptor blocker on diabetic nephritic syndrome patients

目的观察大剂量转换酶抑制剂(ACEI)及受体拮抗剂(ARB)联合治疗糖尿病肾病综合征的疗效并评价其安全性。方法采用随机对照法将51例糖尿病肾病综合征患者分为对照组和治疗组,观察用常规剂量与加用大剂量ACEI及ARB联合治疗的疗效差异,治疗前后监测24小时尿蛋白(UP)、血肌酐(Scr)、血浆白蛋白(ALB)及血脂状况,并进行对照。结果治疗组在提高疗效、降低Scr、减少UP、增加ALB及改善血脂状态方面与对照组相比均有明显统计学意义,且无严重不良反应发生。结论在糖尿病肾病综合征常规治疗基础上,加用大剂量ACEI及ARB联合治疗能明显促进各项指标好转、提高疗效、安全性好、可进一步改善糖尿病肾病综合征的临床预后。

objective To investigate the efficacy and satety of large-dose ACEI and ARB on diabetic nephritic syndrome. Methods 51 patients with diabetic nephritic syndrome were randomly divided into large-dose ACEI and ARB treated group and control group . patients in control group received routine therapy ,While those in treatment group were treated with routine therapy large-dose ACEI and ARB. A series of experimehtal markers were observed before and 8 weeks after the trial. Results After treatment all parameters were improved in both groups There were significant difference in all parameters between treatment and control groups.Conclusions Large-dose ACEI and ARB, With its safety and reliabitity,has been proved clinical benefits to meliorate prognosis in treatment of patients with diabetic nephritic syndrome
