老年人深部真菌感染与耐药现状的临床调查 被引量:2

摘要 目的了解老年患者深部真菌感染及耐药情况。方法收集我院2001-2005年中临床分离检验的老年人深部真菌感染菌株170株,对常见念珠菌进行5种抗真菌药物的体外敏感试验。结果临床老年人深部真菌感染以呼吸道感染最为严重,占110例,其次为肠道26例,泌尿道20例,口咽部5例,血液1例。菌种分布白色念珠菌最多,占55%;其次为热带念珠菌,占 20%,光滑念珠菌,占11%;其他真菌分离率均<10%。对5种常用抗真菌药进行耐药分析发现氟胞嘧啶、两性霉素B对念珠菌相对3种唑类药物具有较低的耐药率(P=0.00)。结论老年人深部真菌感染及耐药性呈逐年上升趋势,唑类抗真菌药物已呈明显耐药,采用有效防治措施刻不容缓。 Objective To moniter gungus infection and resistance drug actuality of our hospital in 5 years. Methods We collected 170 isolates fungi from clinic of our hospital in 5 years and did outside body susceptibility test by 5 anti - fungi drugs for hackneyed isolates. Results The respiraty infection was the most serious of clinicl fungi infection for 110 isolates, the second was iurinarty infection 26, intestinal 20, mouse and pharynx 5, blood 1, csf 1. The C. albicans was the most of bactetium species distribution 55% ,the sencod was C.tropicalis 20% ,Torulopisis glabeata 11% . The other fungi separated rate was all lower than 10% . We had analsised the resistance drugs for 5 hackneyed anti - fungi drugs. We found that flucytosine(5 - FC) and amphotericin B (AMB) had the lower resistance - drug rate than 3 azoles drugs and there was a significant reference between them (P = 0.00). The C. parapsilosis was the most resistance drug Candida, because its resistance drug isolates were larger than 50% for FCZ and ICZ. Conclusion It was no time to delay because that deep gungus infection and resistence drug rate had raised for years and azoles anti - fungi drugs had resistence drug rate significantly.
