经颅多普勒超声与灌注CT测定脑血流储备的比较 被引量:1

Comparison of Transcranial Doppler (TCD) Uitrasonography and Dynamic Computed Tomography (DCT) in Assessment Cerebrovascular Reserve

摘要 目的观察经颅多普勒超声(TCD)和灌注CT在测定脑血流储备上是否一致。方法分别用TCD屏气法和灌注CT口服乙酰唑胺法测定了7例缺血性脑血管病患者13支大脑中动脉支配区的脑血流储备。屏气试验采用正常吸气末平静屏气30s;口服乙酰唑胺试验采用口服乙酰唑胺2.0g,口服乙酰唑胺后2.0~2.5h内行第2次灌注CT检查。结果将试验所得计量资料转换为等级资料,行两配对样本的秩和检验(P=0.039),说明TCD与灌注CT所测得脑血流储备值的差异有显著性意义,不存在一致性;散点图直观地显示TCD与灌注CT所测得的脑血流储备值的之间没有明显的直线相关关系。结论尚未发现TCD与灌注CT在脑血流储备测定上存在显著的一致性,在评价狭窄大脑中动脉的脑血流储备时,TCD尚不能够代替灌注CT。 Objective To compare transcranial Doppler (TCD) ultrasonography and dynamic computed tomography (CT) in assessing cerebrovascular reserve(CVR).Methods Both TCD with breath holding test and dynamic CT with oral ac- etazolamide test were done in seven ischemic cerebrovascular disease patients,total 13 middle cerebral artery.Results Conver- ting measurement data to enumeration data,the Wilcoxon Two-Related-Samples Test showed(P=0.039),which indicated that the difference between CVR assessed by TCD and dynamic CT was significant.The scatterplot showed that there wasn't liner correlation relationship between TCD and dynamic CT in assessing CVR.Conclusion The correlation between CVR assessed by TCD and dynamic CT has not been found.TCD can't replace dynamic CT in assessing CVR of the stenotic middle cerebral artery.
