CT脑池造影术在小儿蛛网膜囊肿的分型诊断及治疗中的应用

CT cisternography in the classification and treatment of childhood intracranial arachnoid cyst

目的探讨CT脑池造影术(CT cisternography,CTC)在小儿蛛网膜囊肿(intracranial arachnoid cyst,IAC)的分型诊断及治疗中的应用价值。方法对19例小儿蛛网膜囊肿患者行CT脑池造影术以确定其是否为交通性。将有明显临床症状和虽无明显症状但CT显示囊肿周围脑叶严重发育不全的非交通性蛛网膜囊肿作为手术指征。术后再次进行CT脑池造影术以评价手术后的治疗效果。结果19例患儿中有6例为交通性蛛网膜囊肿、13例为非交通性蛛网膜囊肿。手术后全部患儿的囊肿均与周围脑池沟通良好,临床症状均有不同程度的改善。结论CTC是小儿蛛网膜囊肿的简单、可靠的诊断方法,可用于判断囊肿的类型,其检查结果可协助筛选适合手术治疗的患者,术后CTC可提供囊肿的动力学变化信息并用于手术效果的初步评价,缺点是它的有创性。
