多节段颈椎病前路颈椎间盘切除和椎体次全切除术的联合应用 被引量:7

摘要 目的 探讨颈椎间盘切除和椎体次全切除术联合应用治疗多节段颈椎病的效果及特点。方法  4 8例多节段颈椎病患者 ,平均年龄 5 7岁 ,累及椎间隙数 3个 ,一期完成单个椎间盘切除和单一椎体次全切除术。术后观察神经功能评分、植骨融合率、畸形矫正和并发症情况。结果 颈椎间盘切除和椎体次全切除术联合应用能有效地解除脊髓的压迫 ,融合率高 ,较好地恢复颈椎生理曲度 ,并发症少 ,神经功能JOA评分均较术前有显著提高。结论 颈椎间盘切除和椎体次全切除术联合应用治疗多节段颈椎病能够达到充分减压的目的 ,解除脊髓前方的压迫 ,可以兼顾减压需要和尽可能多地保留颈椎结构 。 Object To discuss the effect of discectomies and corpectomies used together in multilevel cervical spondylotic myelopathy and its characteristics. Methods 48 cases of multilevel cervical spondylotic myelopathy, average age is 48 years old, counts of vertebral interval involved is 3, were operated with discectomies and corpectomies together. Postoperative neurologic recovery, fusion rates, deformity correction and complication were observed. Results Discectomies and corpectomies used together in multilevel cervical spondylotic myelopathy can effectively decompress spinal cord, restore lordosis of cervical spine, increase fusion rate and decrease complication. JOA score was higher post-operation than pre-operation. Conclusions Discectomies and corpectomies used together in multilevel cervical spondylotic myelopathy can effectively decompress spinal cord, can give attention to decompression and to reserving vertebral structure to the greatest extent and avoiding complication resulting from resecting more one vertebral.
