成人股骨头缺血坏死早期临床表现及CT征象分析

Clinical features and CT manifestation of avascular necrosis of femoral head at early stage in adults

目的:探讨股骨头缺血坏死的早期诊断及CT征象。方法:对39例早期ANFH的临床表现及CT征象进行分析。结果:主要临床表现为髋关节或下肢不适,髋关节活动受限,纵向扣击痛(+),“4”字实验(+)。股骨头主要CT征象有:①股骨头内骨小梁紊乱、模糊、扭曲、硬化;②股骨头内出现囊状软组织密度影,出现“新月征”;③股骨头皮质轻度塌陷,表现为皮质成角、台阶征。结论:CT检查对ANFH的早期诊断有重要意义。

Objective: To study CT manifestation and early diagnosis of avascular necrosis of femoral head in adults Methods: CT manifestations and clinical features of 39 cases of avascular necrosis of femoral head were analyzed Results: Clinical features included discomfort of hip joints and lower limb, restriction of hip joints' movement, portrait-knock-pain (+), palrick test (+). CT manifestations included trabculae turbulence, blur, contortion, sclerosis;cystic low-density areas; crescent sign and light collapse of the femoral head, such as angle in compact bon sidestep sign. Conclusion: CT scanning can find out early stage of avascular necrosis of femoral head.
