老年患者腹腔镜胆囊切除术围手术期的处理

Treatment in the peri - operative period of laparoscopic cholecystectomy for aged patients

目的:总结老年胆囊良性疾病50例腹腔镜胆囊切除术围手术期的处理经验。方法:为50例60岁以上的老年胆囊疾病患者行腹腔镜胆囊切除术。术前除常规检查外,特别注意心、肺功能及血糖的检查和治疗术中监测血气和血氧饱和度;术后侧重观察心肺功能和测定血糖。结果:45例完成腹腔镜胆囊切除术,5例中转开腹;腹腔镜胆囊切除患者术后24h均恢复胃肠道功能,除1例因切口感染外,余顺利康复出院,平均住院5.2d。结论:合理且周密的围手术期处理是保证老年患者耐受腹腔镜胆囊切除术的重要环节。

Objective:To summarize the dealings before and after operation of laparoscopic cholecystectomy in 50 cases. Method: 50 patients over age 60 with benign cholecystopathy were performed laparoscopic cholecystectomy. Before operation, they were examined the heart, lung disease and blood sugar concentration besides the routine examination; in the operation, they were monitored the blood gas and degree of blood oxygen saturation; after operation, they were observed the heart and lung function and blood sugar level. Result: 45 cases were performed laparoscopic cholecystectomy with the gastrointestinal function recovered in 24 hours, all of them were cured except for one case with infection of the incision. The other 5 cases were changed to laparotomy. Conclusion: Rational and thorough dealings in the peri - operative period is very important for old patients to withstand the laparoscopic cholecystectomy.
