阴道用单剂量米索促宫颈成熟的作用研究

Effect of Intravaginally Administering Single Dose of Misoprostol on Cervical Ripening

目的:探讨单剂量米索促宫颈成熟作用。方法:选择宫颈评分≤6分,孕龄≥41周的足月孕妇,将米索50μg置于阴道后穹隆,观察宫颈变化。结果:米索能显著提高宫颈评分,促宫颈成熟有效率达到85.5％,高于对照组10％(P<0001)。用药至临产的时间(12.9±7 8)h,明显短于对照组(52.3±4.4)h(P<0.001),米索组剖宫产率27 6％,明显低于对照组42 9％(P<0 05);两组新生儿APGAR评分无显著差异(P>0.05),并未出现子宫收缩过频,胎心异常等副作用。结论:阴道用单剂量米索50μg能有效使延期妊娠结束分娩,并能避免副作用的发生。

Objective To detemine if cervical ripening resulted from single dose of 50 pg misoprostol administered intravaginally in prolonged pregnant women. Methods One hundred and forty-six women with cervical Bishop score of 6 or less, who were 41 or more weeks pregnant, were divided into both the treatment and control groups. The pregnant women in the treatment (76) and control (70) groups were treated by placing misoprostol and placebo into the posterior vaginal fornix respectively. The change in Bishop score 8 and 24 hours af ter the treatment, the time from medication to labor, the number of vaginal delivery within 24 hours, new born baby' s weight and APGAR score were observed Oxytocin was administered if delivery didn't occur 48 hours after the treatment. Results Sixty patients(78 9% ) in treatment group and 6 patients (8 6%)in control group achieved cervical ripening within 8 hours ( P<0 01) Sixty-five patients (85 5 % ) in treatment group and 7 patients (10% ) in conrtrol group achieved cervical ripening within 24 hours ( P<0.01) Time from treat -ment to labor was (12.9±7 8)h in treatment group, and (52.3±4.4)h in control group ( P<0 01) Vaginal delivery rate within 24 hours was 60.5% (46/76) in treatment group and 5.7% (4/70)in control group ( P<0.01) Conclusion The single dose of 50 pg misoprostol admmistered vaginally is effective for the cervical ripening and labor initiation and has no adverse effeets
