足月妊娠临产前后子宫下段肌层FP、EP_2的表达

摘要 目的 研究前列腺素F2α受体(Prostaglandin F2α Receptor,FP)及前列腺素E2受体亚型-2(Prostaglandin E2 Receptor subtype-2,EP2)在足月妊娠临产前后子宫下段肌层组织中的表达变化,探讨PGF2α、PGE2在分娩发动中的作用机制,为临床有效地防治分娩发动异常所致的疾患提供理论基础。方法81例子宫下段肌层组织标本分别取自足月剖宫产术中,其中未临产组56例(妊娠37—38+6周16例,39—39+6周20例,40-40+6周20例),临产组25例(潜伏期12例,活跃期13例)。通过逆转录-多聚酶链反应(RT-PER),半定量研究各组FP及EP2亚型的mRNA的表达。结果RT-PCR方法的研究表明FP的mRNA的表达在临产组高于未临产组(P=0.003),两组EP2的mRNA的表达量无差异(P=0.38);未临产组不同孕周组之间比较FP、EP2的mR-NA的表达,差异均无显著性(P值均>0.05)。结论足月妊娠37周后,临产的子宫下段肌层的PGF2α的受体FP的表达较未临产时增高,提示PGF2α受体FP的表达上升与分娩的发动有关,EP2的mRNA的表达变化与分娩的发动无明显的相关性。 Objective To determine the effect of gestational age and labor on the expression of relaxatory pros-taglandin E2 receptor subtype - 2 (EP2) and contractile PGF2a receptor (FP) in term pregnant human lower segment myometrium. Methods Biopsy of lower segment myometrium was taken at cesarean from two groups of parturients. One is term yet not in labor with 56 cases including 16 at 37 - 38+6 weeks gestational age, 20 at 39 - 39+6 weeks and 20 at 40 - 40+6 weeks. The other is term in labor of 25 cases. Expression of mRNAs of EP2 and FP receptors were analyzed by reverse transcription - polymerase chain reaction (RT- PCR) . Results The expression of mRNA of contractile FP receptor was significantly higher in parturients in labor than that not in labor ( P = 0.003) . There was no difference of relaxant EP2 receptor mRNA expression between the two groups ( P = 0.38) . Conclusions Increased expression of FP together with increased prostaglandin from intrauterine tissues may lead to the myometrial contraction pattern that occurs during the onset of labor. It indicates a role of uterine FP in mediation of uterine contractility on the initiation of labor. EP2 expressed in the lower segment myometrium is not closely associated with the onset of term labor.
