成人股骨头缺血坏死关节囊(腔)异常CT研究

CT study of abnormal articular capsule of avascular necrosis of femoral head in adult

目的 探讨成人股骨头缺血坏死关节囊 (腔 )异常的出现率、发病机制和CT表现。材料与方法 回顾性分析具有典型X线和CT表现或 /和经病理或随访证实的 138例成人ANFH病人的CT片 ,从中挑选出伴关节囊 (腔 )异常者进行分析。结果 45个出现关节积液和其他关节囊 (腔 )异常 ,包括钙质样高密度游离体 (6个 ) ,关节囊肥厚 (15个 ) ,关节囊钙化(5个 )和髂腰肌囊扩张 (7个 )。所有 45个关节的股骨头均已发生塌陷。结论 股骨头塌陷后 ,股骨头外移、应力改变和对髋关节囊的异常牵拉 ,是成人股骨头缺血坏死关节囊 (腔 )异常的主要原因。不同关节囊 (腔 )异常具有不同的发生率和CT表现。

Purpose To study incidence,pathogenesis and CT manifestations of articular capsule and articular cavity in avascular necrosis of femoral head(ANFH)in adult.Materials and methods CT films of 138 cases with ANFH in adult were read,all having typical X-ray and CT signs or being proved by pathology or follow-up.From them,the cases with abnormal manifestations of articular capsule were chosen and analysed.Results Abnormality of articular capsule appear in 45 hip joints with femoral head out of normal shape,including effussion(45),intraarticular free bodies of calcificative-density(6),thickness of articular capsule(15),calcification of articular capsule(5)and enlargement of iliopsoas bursa(7).Conclusion Abnormality of articular capsule is caused by lateral motion of femoral head,stress change of the jiont and abnormal pull of the capsule.Incidence and manifestation vary in different kind of capsule(cavity)abnormalities.
