疑难交叉配血的探讨及处理方法

Analysis and management of the difficult crossmatching

目的分析交叉配血不合的原因找到处理方法,使病人能及时、有效、安全地进行输血。方法采用手工方法和微柱凝胶法进行血型的鉴定和交叉配血试验,对交叉配血不合者,进行冷凝集检测、二硫苏糖醇处理完全抗体及谱细胞筛选不完全抗体。结果共收集21例交叉配血不合案例,其中由冷凝集素引起的交叉配血不合5例,由完全抗体引起的交叉配血不合2例,由不规则抗体引起的交叉配血不合14例。结论A、B、O、RhD同型输血已可能发生交叉配血不合,应根据引起交叉配血不合的不同原因,用不同方法来处理血浆、血球,可以消除其影响交叉配血不合的因素。

Objective To analyse the reasons of crossmatehing incompatibility before blood transfusion and manage them,then,all pationts can get timely and safely 'helped-blood'.Methods Crossmatching was performed by microcolumn gel test(MGT),and compared with that by manualpolybrene method.If the crossmatching incompatibility,do cryoglobulins test and DTT manage completed antibody and Panel manage discrepancy antibody.Result 5 cases of crossmatching result from cryoglobulins,
