血清Cystatin C评估儿童肾小球滤过功能的价值 被引量:1

Evaluation of Serum Cystatin C as Marker for Glomerular Filtration Rate in Children.

摘要 目的:探讨血清Cystatin C(Scys C)测定在评价儿童肾小球滤过功能中的临床意义。方法:对50例健康儿童(对照组)和55例肾病患儿(肾病组)测定Scys C、血清肌酐(Scr)和内生肌酐清除率(Ccr),比较Scys C与Scr、Ccr的相关性和敏感性。结果:2岁以上健康儿童Scys C的95%可信限的参考范围为(1.05±0.28)mg/L。对照组和肾病组Scys C的差异有显著意义(P<0.01),Scys C与Scr有显著正线性相关(P<0.01)。1/Scys C与Ccr相关性比1/Scr与Ccr的相关性要好。Logistic回归分析表明,Scys C浓度仅与GFR有关,而与受试者的年龄、性别、身高和体重无关。ROC分析得到Scys C最佳截点为1.30mg/L,据其诊断GFR下降的敏感性为95%,特异性为90%,准确性为93%。结论:Scys C反映儿童肾小球滤过功能不受年龄、性别、身高和体重等肾外因素的影响,诊断GFR下降的敏感性、特异性和准确性高于Scr和身高/Scr,是一种较理想的评估儿童GFR的内源性标志物。 Objective To investigate whether evaluation of serum cystatin C level can be a reliable,sensitive and safe method to determinate glomerular filtration rate(GFR) in children.Methods 55 pediatric patients(31 males and 24 females,aged 2 to 16) with different renal functions were all examined by three kinds of methods,which were clearance of serum creatinine(Ccr)(4 h urine method) level,serum Cystatin C level,and serum creatinine level(Scr) respectively.
