血管移位在复杂断指再植中的应用

The Application of Blood Vessel Transposition for Replantation of Amputated Finger in Complex Disjunct Finger

目的:探讨血管移位在复杂断指再植中的临床疗效。方法:22例22指复杂性离断指患者,近端指动脉毁损,采用邻近指动脉移位来恢复离断指体远端血运的再植方法,术后常规抗感染、解痉、扩血管及抗凝等治疗。结果:再植指成活率为95%,1例1指在术后2 d坏死;随访患者5～12月,平均8月,再植指外观好,两点辨别觉4～8 mm,痛温觉及捏持功能恢复;断指再植功能评定:优12指,良8指,差1指,优良率达95%。结论:对复杂性指体离断伤患者,应用邻指指固有动脉移位为再植提供一种理想且有效的治疗方法。

Objective:To investigate the application of blood vessel transposition for replantation of amputated finger in complex disjunct finger. Methods: 22 patients with 22 cases complex disjunct fingers were involved in the study. All patients were treated with transfer of proximal digital artery of border upon finger to rebuild the distal blood circulation of trauma finger. Anti-inflammatory, spasmolysis, dilatation of a blood vessel, anticoagulation and so on were adopted in post-operation. Results: The survival rate was 95%. 1 fingers in 1 case was necrotic in 2 days after operation. All cases were follow-up 5 to 12 months with averaged 8 months. The shape of replanted fingers were good, two points discrimination was 4 to 8 ram. There were 12 fingers with excellent effects, 8 fingers with good effects and 1 finger with bad effects, the fineness rate was 95%. Conclusions: It is an ideal and effective choice to transfer of proximal digital artery of border upon finger to rebuild the distal blood circulation of trauma finger in replantation of amputated finger for complex disjunct finger.
