急性坏疽性阑尾炎根部处理41例临床分析

Clinical Analysis of Management at the Base of 41 Patients with Acute Gangrenous Appendicitis

目的:探讨坏疽性阑尾炎根部不同处理的效果及体会。方法:回顾性分析我院2001年9月至2008年11月手术治疗的41例急性坏疽性阑尾炎患者临床资料。结果:31例伤口一期愈合,10例二期愈合。结论:急性坏疽性阑尾炎手术应根据阑尾根部坏疽情况选择不同处理方式。

Objective: To study the efficacy of management at the base of acute gangrenous appendicitis. Methods: 41 cases of acute gangrenous appendicitis with undergoing appendectomy were retrospectively analyzed from Sep 2001 to Nov 2008. Results:9 case with no perforation were received pursestring,21 cases with base necrosis mattress suture and cover with epiploic appendices, 6 cases with necrosis and perforation at the base of appendix received appendicieal stump embedded into cecum and lembert suture, 5 cases with necrosis in the whole base of appendix received removed appendix and interrupted seromuscular suture covered with epiploic appendices, pediculated epiploon and lateral peritoneal. 31 cases healed by first intention and 10 secondary healing. Conclusion: The various methods with treatments should be adopted to manage the base of acute gangrenous appendicitis according to the pathological types.
