摘要
目的探讨影响肺腺鳞癌预后的因素,为改进治疗方案奠定基础。方法对361例肺腺鳞癌患者的临床资料进行回顾性分析。选择性别、年龄、症状、病程、肿瘤部位、X线分型、肿瘤边缘、分叶、密度、手术方法、胸膜皱缩、大体标本侵及胸膜或胸壁情况、手术性质、肿瘤最大直径、淋巴结转移模式、转移度(阳性淋巴结个数/淋巴结总数)、分化程度、病理侵及胸膜或胸壁情况、病理T项、病理N项、病理分期、术后放疗、术后化疗、术后复发转移等24个非重复因素进行预后分析,生存率计算采用寿命表法。统计分析运用Log-rank检验和Cox模型多因素分析。结果本组腺鳞癌根治术后的5年、10年生存率分别为29.1%和15.4%。Log-rank检验示X线分型、手术方法,大体标本侵及胸膜或胸壁情况,手术性质、肿瘤最大直径、病理侵及胸膜或胸壁情况、病理T项、病理N项、淋巴结转移模式、转移度、病理分期、术后复发转移及术后放疗对预后有显著影响(P<0.05)。经Cox多因素分析示术后复发转移、病理分期、手术性质因素对预后影响最大。结论肺腺鳞癌是一种生物学行为恶性程度较高的非小细胞肺癌,单纯外科治疗效果尚不满意。
Objective To investigate the clinic-pathologic characteristics and the prognostic factors in the patients with lung adenosquamous carcinoma.
