摘要
目的探讨人血浆瘦素水平与肥胖程度、脂肪分布的关系。方法对100例肥胖者(男性52例,女性48例)和71例非肥胖者(男性38例,女性33例)用免疫放射法测定空腹血浆瘦素水平,同时测量身高、体重、腰围、臀围,根据身高、体重计算体重指数。对其中19例肥胖者(男12例,女7例),15例非肥胖者(男6例,女9例)经CT扫描测量腹部内脏和皮下脂肪面积。通过分析瘦素与人体测量指标的相关性,初步探讨瘦素与肥胖及脂肪分布的关系。结果空腹血浆瘦素水平性别差异显著(P<0.001),女性是男性的(2~3)倍;瘦素与体重指数呈正相关(男r=0.6772,P<0.01;女r=0.7191,P<0.01),与腰围正相关(男r=0.7384,P<0.01;女r=0.5818,P<0.01);男性血瘦素浓度与腰/臀比值呈正相关(r=0.5556,P<0.01),而女性无相关性(r=0.1871,P>0.05)。经CT扫描测量皮下和内脏脂肪面积,发现瘦素与皮下脂肪面积的相关性高于与腹部内脏脂肪面积的相关性。结论瘦素水平不仅与肥胖程度有关,还与脂肪分布有关,即与皮下脂肪的相关性更加密切。
Objective To investigate the relationship between human plasma leptin concentrations and obesity as well as body fat distribution. Methods We measured fasting plasma leptin concentrations with IRMA of 100 obese (including 52 men and 48 women) and 71 non-obese (including 38 men and 33 women). Physical exami- nation included measurement of height, body weight as well as abdominal circumfer- ence and hip circumference. Body mass index (BMI) was calculated by height and body weight. We used CT to evaluate subcutaneous and intra -abdominal fat area of 19 normal weight persons (including 12 men and 7 women) and 15 obese (including 6 men and 9 women). Results Fasting plasma leptin levels are 2~3 times in fe- male than in male, the difference is significantly (P<0.001). Leptin values are correlated with BMI(r=0.6772, P<0.01 in male and r=0.7191, P<0.01 in fe- male) and abdominal circumference (r=0.7384, P<0.01 in male; r=0.5818, P< 0.O1 in female). Leptin concentrations are correlated with WHR in male (r= 0.5556, P<0.01), while this correlation is not exist in female (r=0.1871,P> O.05). Leptin levels are higher correlated with subcutaneous fat areas than correlated with intra -abdominal fat areas. Conclusions Leptin levels are associated with fat content and fat distribution.
