摘要 目的 探讨婴幼儿人工耳蜗植入术后调试方法。方法 对79名接受了人工耳蜗植入手术的婴幼儿测试电极阻抗,通过使用主观判断法和客观推测法进行术后调试。结果 电极阻抗测试中74例全部正常(93.67%),3例一个电极异常(3.80%),2例两个电极异常(2.53%)。调试中,1例第13、14号电极引起颜面部抽动,1例第4-22号电极无声反应,电极植入失败。72例使用主观判断法,6例在开机初期使用客观推测法,一、两个月后改用主观判断法进行测试。40例在开机一月内,38例在开机两、三月后建立起听性条件反应。声场啭音测试患儿听阈都在言语香蕉图上限,对言语的大声和响器的大声无不适感觉。问卷调查显示开机后很快即对各种小声音反应灵敏,语言发展进步明显。结论 婴幼儿人工耳蜗植入术后调试采用主观判断法和客观推测法,以主观判断法为主,而术前助听器试戴、术后康复训练是调试准确的重要保证。 Objective To discuss the methods of mapping in cochlear implant for young children. Methods The behavioral observasion and obejective method were used to mapping in 79 young deaf children after the electrode impedance test. Results 74 patients obtained normal implant function. 1 electrode in 3 patients and 2 electrodes in 2 patients were abnormal,72 patients used behavioral observation and 6 patients used obejective method to mapping. The accurate hearing responses was obtained by 40 patients in a month and-38 patients in two or three months. Hearing threshold was shown in upper part of the speech spectrum. Investigation showed they did not feel discomfort when they heard loudsound. Their responses to many kind of undertone was sensitive quickly after switch on the devices. They had significant progress hearing and speaking. Conclution The methods of mapping in cochlear implant for young deaf children include behavioral observasion and obejective method, but the behavioral observasion is more important.
