闭合性肝损伤117例诊治分析

Analysis of diagnosis and treatment in the 117 cases with closed liver injury

目的提高基层医院对闭合性肝损伤的诊治水平。方法对我院1991年7年～2002年7月收治117例闭合性肝损伤患者的诊断和治疗进行回顾性分析。117例中,非手术治疗12例,手术治疗105例。结果108例治愈,9例死亡。结论在严格掌握适应证和密切动态观察伤情变化的条件下,非手术治疗闭合性肝损伤是安全可行的。手术治疗应根据伤情合理选择并正确使用不同的术式,重度肝损伤时常常需联合应用几种术式才能获得良好的效果。

Objective To improve the diagnosis and therapeutic level of closed liver injury in the basic hosptal.Methods From July1991to July2002 ,117cases with closed hepatic trauma were analyzed retro-spetively.Among117cases ,12cases underwent nonoperative treatment ,105cases underwent operative treatment.Results108cases were cured and9cases died.Conclusion Non operative treatment of the close liver injury was safe and practiceable when the indications were strictly controlled and changes of traumatic condition are closely observed.The operative treatment should be selected reasonably and different operative manners should be used correctly according to the traumatic status.For taking a good efficacy ,a conbined operative manners is often neccesary in the severe hepatic trauma.
