肱-踝动脉的脉搏波传导速度、超敏C反应蛋白在高血压合并颈动脉硬化早期诊断中的价值 被引量:3

摘要 目的:探讨肱-踝动脉的脉搏波传导速度(baPWV)、超敏C反应蛋白(hsCRP)在高血压合并颈动脉硬化早期诊断中的价值。方法:选择150例高血压合并颈动脉硬化患者,按照高血压分级分为A、B、C三组;采用高分辨血管外超声法测量颈动脉内膜与中膜厚度(IMT),分为内膜增厚组和斑块组。应用动脉硬化测定仪测定baP-WV;采用胶乳免疫增强比浊法测定血清中hsCRP。结果:baPWV值依序为C>B>A(P均<0.05);而三组hsCRP无显著差异(P>0.05);斑块组baPWV、hsCRP均高于内膜增厚组(P<0.05)。baPWV与hsCRP水平呈正相关(r=0.54,P<0.05)。结论:baPWV与高血压水平、动脉硬化程度密切相关,可以早期反映血管的弹性改变,而hsCRP仅与动脉硬化程度密切相关,提示baPWV联合hsCRP在一定程度上能够早期预测和防治动脉粥样硬化。 Objective To explore the value of early diagnosis of brachial-ankle pulse wave velocity in the artery(baPWV),high sensitivity c-reactive protein(hsCRP)in patients with hypertension and carotid atherosclerosis.Methods Total 150 patients with hypertension and carotid atherosclerosis were selected and divided into three groups based on the degree of hypertension(group A,B,C).The thickness of carotid artery intima and media tunica(IMT)were measured with high-resolution ultrasound,then the patients were divided into endometrial thickness and plaque groups.The serum level of hsCRP was determined with the turbidimetry test of latex immunoenhancement.Results The values of baPWV in three groups was in the following order:group C>group B>group A,(P0.05).The value of baPWV and serum level of hsCRP in patients of endometrial thickness group were both significantly higher than those of plaque group,(all P<0.05).The value of baPWV was positively correlated with the serum level of hsCRP,(r=0.54,P<0.05).Conclusion The value of baPWV was closely related to the degree of atherosclerosis and hypertension,which could reflect the flexibility changes of blood vessels in the early.But the serum level of hsCRP was only correlated with the degree of atherosclerosis.On the whole,baPWV combined with hsCRP could be valuable for predictive diagnosis and treatment in patients with atherosclerosis in early.
