摘要
目的评价实时剪切波弹性成像(SWE)在乳腺组织及其病变中的组织定征能力和实用价值。方法对168例女性患者共215个病灶术前行SWE测定,以穿刺活检或手术病理为诊断金标准,获得乳腺病灶的弹性模量值;同样方法获得178例正常乳腺腺体及108例乳腺皮下脂肪的弹性模量值。结果乳腺恶性病灶(47个)的平均弹性最大值、平均值及最小值分别为(113.92±61.05)kPa、(55.68±31.65)kPa及(23.45±16.13)kPa,良性病灶(167个)的平均弹性最大值、平均值及最小值分别为(37.26±24.10)kPa、(23.45±14.73)kPa及(13.61±9.49)kPa,良恶性病灶3个弹性值比较差异均有统计学意义(P<0.01);乳腺腺病(61个病灶)的3个弹性值均高于纤维腺瘤(69个病灶,P<0.05);而纤维腺瘤的弹性值显著高于正常乳腺腺体和脂肪组织(P<0.01)。结论 SWE可直接实时检测各种组织的弹性模量,弹性量化数据的大小显示出SWE具有较高的组织定征能力和鉴别诊断价值。
