甲状腺乳头状癌组织中瘦素的表达与血管生成及转移的关系 被引量:18

The expression of leptin in thyroid papillary carcinoma and its effect on intratumor angiogenesis and metastasis

摘要 背景与目的:瘦素(leptin)能通过与其受体OB-R结合,活化细胞内多条信号转导通路,在许多生物学效应中起着重要作用。近年来研究发现leptin可参与部分恶性肿瘤的发展。本研究探讨leptin在甲状腺乳头状癌组织中的表达与血管生成及转移之间的关系和意义。方法:应用免疫组织化学染色法检测58例甲状腺乳头状癌(其中已发生淋巴结转移为29例,未发生淋巴结转移为29例)和26例甲状腺腺瘤组织中leptin与CD105(标记血管密度)的表达情况。结果:甲状腺乳头状癌组织leptin表达阳性率为91.38%,显著高于甲状腺腺瘤组织(7.70%,χ2=56.07,P<0.05)及癌旁组织(5.17%,χ2=88.94,P<0.05)。甲状腺乳头状癌已转移组中leptin表达(93.10%)强于未转移组(89.66%,χ2=10.68,P<0.05)。甲状腺腺瘤组织与癌旁组织leptin表达差异无统计学意义(χ2=2.31,P>0.05)。甲状腺乳头状癌已转移组新生血管密度(59.92±18.54)高于未转移组(22.44±14.50,P=0.000)、甲状腺腺瘤组(4.18±2.14,P=0.000)及癌旁组(2.03±1.52,P=0.000);甲状腺乳头状癌未转移组新生血管密度高于甲状腺腺瘤组(P=0.005)及癌旁组(P=0.000);甲状腺腺瘤组与癌旁组间新生血管密度差异无统计学意义(P=0.919)。甲状腺乳头状癌组织中新生血管密度随leptin表达的强度增强而增加(r=0.88,P<0.05)。结论:leptin参与甲状腺乳头状癌组织中血管的生成及肿瘤的发展。 Background and purpose:Leptin acts via its receptor OB-R to activate many signaling pathways.Recent research indicates that leptin participates in the development of some neoplasma.This study investigated the expression of leptin in thyroid papillary carcinoma tissues and analyzes the role of leptin in tumor angiogenesis and metastasis.Methods:Fifty-eight cases of thyroid papillary carcinoma(29 metastasized,29 non-metastasized),and 26 cases of thyroid adenoma pathologically confirmed and were included in this study.Immumohistochemical staining(SP method) was performed to detect the expression of leptin and CD105(to detect microvessel density,MVD) in the thyroid papillary carcinoma,thyroid adenoma and para-tumor tissues.Results:Leptin was highly expressed(total positive rate of 91.38%,and metastasized rate of 93.10%,and non-metastasized rate of 89.66%,χ2=10.68,P<0.05) in thyroid papillary carcinoma,whereas the positive rate of leptin was 7.70% in thyroid adenoma(χ2=56.07,P<0.05) and 5.17% in para-tumor tissues(χ2=88.94,P<0.05),respectively.There were no differences between the adenoma group and para-tumor tissue(χ2=2.31,P>0.05).The MVD in the thyroid papillary carcinoma metastasized group(59.92±18.54) was higher than that in the non-metastasized group(22.44±14.50,P=0.000),thyroid adenoma group(4.18±2.14,P=0.000) or para-tumor tissue group(2.03±1.52,P=0.000).The MVD in the thyroid papillary carcinoma non-metastasized group was higher than that in the thyroid adenoma group(P=0.005) and para-tumor tissue group(P=0.000).No differences were found between the adenoma group and para-tumor tissue group(P=0.919).There was a positive correlation between MVD and the expression of leptin(r=0.88,P<0.05).Conclusion:This study suggests that increased expression of leptin in thyroid papillary carcinoma contributes to tumor angiogenesis and the development of neoplasm.
