高顺应性球囊治疗症状性脑血管痉挛

Treatment of symptomatic cerebral vasospasm by highly compliant balloon

目的总结高顺应性球囊治疗蛛网膜下腔出血后症状性脑血管痉挛的诊疗经验。方法回顾性分析采用高顺应性球囊治疗的12例症状性脑血管痉挛病人的临床资料。本组检出重度痉挛血管共18支。结果手术成功率100%,残余狭窄0~30%,平均12%。痉挛明显改善10例(83.3%),无变化1例,短期症状反复1例。术后无操作相关并发症发生。出院时按照改良Rankin评分:1分7例,2分3例,3分1例,6分1例。结论使用高顺应性球囊治疗症状性脑血管痉挛安全有效,临床缓解率高。

Objective To summarize the experience with highly compliant balloon for treatment of symptomatic cerebral vasospasm after cerebral subarachnoid hemorrhage.Methods The clinical data of 12 patients with symptomatic cerebral vasospasm who underwent the treatment with highly compliant balloon were analyzed retrospectively.Severe vasospasm was detected in 18 vessels.Results The surgical success rate was 100%.The residual stenosis was 0 to 30% and 12% on average.The vasospasm was improved in 10 patients(83.3%),remained unchanged in 1 and recurred within short-term in 1.No operation-associated complication was found after operation.Modified Rankin scores at discharge were as follows: 1 in 7 cases,2 in 3,3 in 1 and 6 in 1.Conclusions Use of highly-compliant balloon is a safe and effective means for symptomatic cerebral vasospasm and can get high clinical remission rate.
