原发性高血压患者高敏C-反应蛋白及尿微量白蛋白/尿肌酐比值的相关性 被引量:11

Relationship of high-sensitive C-reactive protein with urine microalbumin/creatinine in essential hypertension

摘要 目的探讨高敏C-反应蛋白(hs-CRP)是否影响高血压患者尿微量白蛋白/尿肌酐比值(ACR)的变化。方法对入选的166例新近诊断为原发性高血压的患者,以及50例健康对照组,行血清hs-CRP、脂联素、尿酸、尿微量白蛋白、尿肌酐等检测,并计算ACR值。原发性高血压患者按照ACR值分为尿微量白蛋白组(ACR≥30mg/g,38例)和尿蛋白正常组(ACR<30mg/g,128例),对两组之间及与对照组之间进行hs-CRP、脂联素等比较,然后以Logistic多因素回归分析与ACR升高的相关因素。结果 (1)健康对照组、尿蛋白正常组、微量白蛋白组血清lg(hs-CRP)分别为(0.04±0.14)mg/L、(0.16±0.09)mg/L和(1.52±0.19)mg/L,两两比较P均<0.05。(2)ACR与hs-CRP的Pearson线性相关分析示:hs-CRP与ACR的相关系数为0.459(P<0.05);Logistic多因素回归分析显示:hs-CRP与ACR的升高独立相关。结论 hs-CRP水平与ACR呈明显正相关,hs-CRP可能是原发性高血压患者ACR升高的独立预测因子。 Objective To study the relationship between high-sensitive C-reactive protein(hs-CRP) and urine microalbumin/creatinine ratio(ACR) in patients with essential hypertension.Methods Serum hs-CRP,adiponectin and ACR were examined in 166 patients with essential hypertension and in 50 healthy persons(healthy group).According to ACR values,166 patients were divided into two groups:microalbuminuria group(n=38,ACR ≥30 mg/g) and normal urine protein group (n=128,ACR<30 mg/g).hs-CRP,adiponectin and ACR were compared among microalbuminuria group,normal urine protein group and control group.Linear correlation analysis was used to find the correlation between variables and Logistic binary regression analysis was used to find the independent factor related with high level of ACR.Results (1) The concentration of lg(hs-CRP) was significantly increased in microalbuminuria group compared with normal urine protein group and healthy group[(1.52±0.19)mg/L,(0.16±0.09)mg/L and (0.04±0.14)mg/L,respectively,P<0.05].(2) Pearson linear correlation analysis showed that the correlation coefficient between CRP and ACR was 0.459,P<0.05.The Logistic multi-factor regression analysis showed that the hs-CRP was an independent predictor of high level of ACR.Conclusions hs-CRP is positively related with ACR.hs-CRP may be an independent predictor of high level of ACR.
