中国妇女孕前肥胖与不良妊娠结局关系的Meta分析

The Meta-analysis for relationship between pre-pregnancy obesity and adverse pregnancy outcomes

目的探讨孕前肥胖与不良妊娠结局的关系。方法检索中国期刊网及万方数据库1980年至2010年有关孕前肥胖与妊娠结局关系的研究,并采用RevMan4.2.10版本对此进行了定量综合分析。结果经检索筛选后纳入的有关孕前或者体重超标与妊娠结局关系的文章24篇,经异质性检验,分别采用固定效应模型或随机效应模型进行定量综合分析综合。结果表明孕前超重及肥胖增加巨大儿、妊娠期糖尿病及妊娠期高血压疾病发生风险,OR值依次为3.08(95%CI:2.58～3.69)、2.64(95%CI:2.24～3.13)、2.54(95%CI:2.11～3.05)。结论孕前肥胖者发生妊娠期高血压、糖代谢异常及巨大儿的风险明显高于正常体重妇女。

Objective To investigate the correlation between pre-pregnancy obesity and adverse pregnancy outcomes.Methods The literature on the relationship between pre-pregnancy obesity and adverse pregnancy outcomes published in China Journal Net database from 1980 to 2010 were searched and collected,and a comprehensive quantitative analysis for which was carried out by RevMan 4.2.10 version.Results There was 24 studies about the relationship between pre-pregnancy overweight or obesity and pregnancy outcome.Since significant heterogeneity were found in these studies by heterogeneity test,random effects model was used for quantitative analysis of integrated comprehensive.The results showed that pre-pregnancy overweight or obesity had increased the risk of macrosomia,gestational diabetes mellitus and hypertensive disorders in pregnancy,the OR were 3.08(95% CI:2.58-3.69),2.64(95% CI: 2.24-3.13),2.54(95% CI: 2.11-3.05).Conclusions The risk of macrosomia,gestational diabetes mellitus and hypertensive disorders in pregnancy is significantly higher in pre-pregnancy obesity women than normal weight.
