原发性结外淋巴瘤在耳鼻及咽部的临床表现特点

The clinical characteristics of primary extra-nodal malignant lymphoma in ear.nose and pharynx

目的:总结原发性结外淋巴瘤在耳鼻及咽部的临床表现,以提高及时确诊率。方法:回顾性分析15例原发性结外淋巴瘤在耳鼻及咽部的临床表现特点,特别是中耳罕见的淋巴瘤,少见的副鼻窦淋巴瘤的临床表现。结果:中耳淋巴瘤1例,以伴有疼痛的慢性分泌性中耳炎(传导性聋)、轻度面瘫为特点;7例扁桃体淋巴瘤,以咽部异物感及单侧扁桃体大为特点;2例鼻及副鼻窦淋巴瘤以血涕、鼻臭、下鼻甲粘膜粗糙,增厚为特点;鼻咽部淋巴瘤以血涕、头痛为特点;口咽部淋巴瘤以咽痛、发热、软腭溃疡、口臭为特点。结论:在发现耳鼻咽部病变时,掌握原发性结外淋巴瘤在耳鼻及咽部的临床表现特点,及时行病理检查,是及时确诊的关键。

Objective:To evaluate the clinical characteristics of primary extra-nodal malignant lymphoma in ear, nose and pharynx,in order to improve correct diagnosis.Method:Clinical characteristics of primary extra-nodal malignant lymphoma in ear,nose and pharynx were retrospectively analyzed in 15 patients.Result:The clinical feature of extra-nodal lymphoma in middle ear one patient consisted of otalgia,chronic secretory otitis media(con- ductive hearing loss),and facial nerve palsy.Lymphomas were located in tonsils in seven cases.The characteristic signs of tonsil lymphoma were unilateral tonsil enlargement and foreign body sensation in the pharynx.Lymphoma was found in nose and accessory sinus in two cases,and it characteristic signs were bloody and odorous rhinorrhea with hypertrophy of the inferior turbinate.The patients suffering from nasopharygeal lymphoma always com- plained of head ache and bloody rhinorrhea.Oropharyngeal lymphoma showed the symptoms of pharyngalgia,re- ves,with soft palate ulces and fetor oris,Conclusion:The key to make a correct diagnosis of extra-nodal lymphoma in ear,nose and pharynx is to get familiar with the clinical characteristics and perform a biopsy promptly.
