重组人表皮生长因子凝胶对动物腹部手术切口抗张强度影响的实验研究

摘要 目的:探讨重组人表皮生长因子(rhEGF)凝胶对动物腹部手术切口的修复作用。方法:实验分两部分进行。第一部分,SD 大鼠90只,随机分为三组(切口内外用药组、切口仅外部用药组和空白基质组),每组30只,每日切口处涂药一次。术后第4、5、6、8、10天每组分别处死6只动物,取切口处皮肤进行拉力试验并行病理组织学检查;第二部分,实验用小型猪6头,设四个用药组,采用自身对照(切口内外用 rhEGF 凝胶组、仅外用rhEGF 凝胶组、rhEGF 水剂组和空白基质组),用药方法同上,分别在术后第5、7、9天各处死2只动物,取切口处皮肤同上法测定和检查。结果:rhEGF 凝胶外部用药组两种动物切口处皮肤抗张强度及拉伸率在不同的用药时间内较空白基质对照组均明显增强(P<0.05,P<0.01);首次内、外用药组的抗张强度明显低于外部用药组(P<0.05,P<0.01),且切口处炎性细胞弥漫性浸润,并见异物巨细胞反应;rhEGF 水剂组抗张强度和拉伸率与rhEGF 凝胶首次内外用药组相当,但低于 rhEGF 凝胶外部用药组。结论:rhEGF 凝胶可明显增强动物手术切口皮肤抗张强度,提高手术切口的愈合质量,加速手术切口的愈合。rhEGF 凝胶不同的用药方式及 rhEGF 不同的剂型对动物皮肤切口抗张强度有一定影响。 Objective:To study the effect of recombinant human epidermal growth factor(rhEGF) gel on abdomi- nal incision in animals.Methods;In experiment Ⅰ,ninety SD rats were randomly divided into three groups.In the treatment group 1 (TG1) rhEGF gel was applied on both inside and outside of the incision,and in the treatment group 2 (TG2) rhEGF gel was applied on the outside of the incision.In the control group (CG),the insision was treated with vehiele.In experiment Ⅱ;four 5cm incisions on both sides of the abdomen were made in each of 6 pigs.The incisions on each pig was treated differently.In one incision rhEGF gel was applied both inside and out- side of the incision (TG1);on second incision by rhEGF gel was applied outside of the incision (TG2);on the third incision rhEGF solution was applied outside of the incision (TG3);in the fourth incision,only vehicle was applied (CG).Results;The tensile strength of the healed incisions of TG2 was remarkably stronger than CG and TG1 in both rats and pigs at different intervals (P<0.05 or P<0.01).The tensile strength of wounds of TG1 was remarkably lower than TG2 (P<0.05 or P<0.01).In the incision diffuse infiltration of inflammatory cells and foreign-body giant cell reaction were observed.The tensile strength and rate of stretch in TG3 were similar to those of TG1 and lower than those of TG2.Conclusion;rhEGF gel can remarkably increase the tensile strength of abdominal incision in animals and promote the healing of incision.
