胰岛素泵在严重烧伤糖尿病患者治疗中的应用

The application of insulin pump in treatment of severely burned diabetic patients

目的：探索严重烧伤糖尿病患者快速控制血糖的新方法。方法：近年来我科收治此类病人共19例,其中10例行胰岛素泵连续皮下输注治疗(CSII组)；另9例采用多次皮下注射胰岛素治疗(MSII组),目标是将血糖基本控制在5～10mmol/L范围内。结果：CSII组与MSII组比较,前者血糖波动性小,低血糖发生率低,微粒皮存活率高,治愈时间明显缩短。结论：CSII法更有效控制严重烧伤糖尿病患者的血糖,提高植皮成活率。

Objective:To explore a way to control the blood glucose level in satisfactory range in diabetic patients with burn injury.Methods:19 diabetic patients with major burns were admitted to our hospital in recent years.In- sulin was administered by two delivery methods:continuous subcutaneous insulin infusion (CSII group,n=10) and multiple-subcutaneous insulin injection(MSII group,n=9).Results:In patients in CSII group a more stable blood glucose level was maintained,with a lower incidence of hypoglycemia and a higher skin graft survival with shorter wound healing time.Conclusion:The insulin delivery method in CSII more effectively controlled fluctuation of blood glucose level and it seemed to improve the survival rate of skin grafts in wounds of diabetic patients with major burns.
