摘要 对于慢性乙型肝炎(chb),目前抗病毒治疗是唯一能够最大限度抑制病毒复制和延缓病情进展的方法.在抗病毒药物中,核苷(酸)类药物对chb的疗效已得到肯定.但目前所有口服核苷(酸)类药物,包括拉米夫定、阿德福韦酯、恩替卡韦、替比夫定等在治疗过程中均可出现不同程度的耐药性.耐药株的出现给乙型肝炎的治疗带来了极大的困难,但这些耐药株是在用药之前就存在还是由于用药而诱发的呢?这些核苷(酸)类药物在应用过程中亦有原发无应答现象,其原发无应答的原因何在?是否其感染的hbv本身就存在耐药株呢?在选用核苷(酸)类药物抗病毒之初进行hbv耐药位点检测,根据检测结果选择有效的抗病毒药物,可以获得较好的疗效,也可以避免不必要的浪费.
