应用瘤内切瘤技术切除大型、巨大型脑肿瘤的临床研究

Resection of Large and Huge Cerebral Tumors by Technique of Resection within Tumors

目的探讨应用瘤内切瘤技术切除大型脑肿瘤的临床效果。方法采用瘤内切瘤技术切除50例直径4 cm～7 cm的大型脑肿瘤。结果45例获全切,5例获次全切。术中出血平均不超过400 ml,平均输血200 ml,无手术死亡。1例发生瘤腔积血再手术清除,术后出现面神经瘫痪2例,3月后康复。近平均18个月随访,复发4例。结论在切除大型脑肿瘤时应用瘤内切瘤技术,肿瘤全切除率高,同时显著减少术中重要神经血管结构损伤,临床效果良好。

Objective To study the clinical effects on resection of large cerebral tumors by the use of the technique of resection within tumors.Methods We resected 50 large tumors with the diameter of 4 cm～7 cm,by the technique of resection of tumor within tumor.Results 45 underwent total resection,5 had subtotal resection.During operation average amount of bleeding was no more than 400 ml,and average amount of blood trasfusion was 200 ml without operating death.One case developed hematocele in tumor cavity and underwent operating clearance.Two cases had facial paralysis after operation and recovered 3 months later.Following up was made for average 18 months.Four cases had relapse.Conclusion There is a high rate of total resection of tumors by the use of minimally invasive technique for resection of large cerebral tumors.It is able to reduce damage of nerves and blood vessels during operation with good clinical effects.
