血清miR-21在诊断宫颈磷癌淋巴结转移中的价值 被引量:4

摘要 目的研究血清miR-21在诊断宫颈磷癌淋巴结转移方面的价值。方法选择2011年1月~2011年9月在广西医科大学附属肿瘤医院妇瘤科住院接受治疗的80例患者为研究对象,其中宫颈癌伴淋巴结转移者25例,不伴淋巴结转移者40例,正常对照组15例。治疗前留取患者血清,实时荧光定量PCR测定血清miR-21水平,Spearman秩相关分析血清miR-21水平与患者淋巴结转移的关系,绘制受试者工作曲线(ROC),计算血清miR-21的诊断界值。结果宫颈癌伴淋巴结转移患者血清miR-21水平较不伴淋巴结转移患者和正常对照组明显升高,差异有统计学意义(P<0.05)。血清miR-21水平与宫颈癌淋巴结转移呈正相关(r=0.408,P<0.001);血清miR-21在宫颈癌确诊病例中诊断淋巴结转移的ROC下面积为0.770,最大约登指数0.485,相应灵敏度56%,特异度92.5%,对应诊断界值6.65。结论血清miR-21在诊断宫颈癌淋巴结转移方面可能有一定的临床价值和应用空间。 Objective To study the expression level of serum microRNA-21(miR-21) in cervical cancer patients and assess the function of serum miR-21 in diagnosing lymph node metastasis(LNM) in cervical cancer.Methods We selected 80 cervical cancer patients hospitalized at the Department of Gynecologic Oncology,Guangxi Tumor Hospital,between January and September 2011.Among them,25 had cervical cancer with LNM,40 had cervical cancer without LNM,and 15 were normal controls.The expression level of serum miR-21 was examined by quantitative reverse transcription polymerase chain reaction(qRT-PCR) and Pearson correlation analysis;ROC curve was drawn to find the most suitable cut-off point.Results The serum miR-21 level of cervical cancer patients with LNM was significantly higher than that in those without LNM and normal controls(P<0.05).The serum miR-21 level was correlated with LNM in cervical cancer(Spearman correlation coefficient = 0.408,P<0.001).In the patients with confirmed LNM,the area under the curve was 0.770 and the maximal Youden index was 0.485.The sensitivity and specificity of serum miR-21 were 56% and 92.5%,respectively;the cut-off point was 6.65.Conclusion Serum miR-21 may be used to estimate LNM and could be a useful tool in making therapeutic decisions in cervical cancer.
