卵巢早衰患者卵巢大小及血流动力学参数与性激素水平的相关性 被引量:18

Relevant research of ovarian diameter,blood index parameters and sex hormone level in premature ovarian failure

摘要 目的探讨卵巢早衰(POF)患者卵巢大小及血流动力学参数与性激素水平的相关性。方法回顾性分析我院32例临床及实验室诊断为POF的患者(POF组)和40名正常体检育龄妇女(健康对照组)的临床资料。所有研究对象均应用经阴道彩色多普勒超声测量了卵巢最大平面平均直径(MOD)、窦卵泡数(AFC)和卵巢间质动脉峰值流速(PSV)、阻力指数(RI),并检测了其血清雌二醇(E2)、基础卵泡刺激素(FSH)和黄体生成素(LH)水平。结果 POF组MOD、AFC、PSV、RI、E2、FSH、LH分别为(18.10±4.93)mm、0个、(6.12±2.60)cm/s、(0.67±0.32)、(72.20±10.45)pmol/L、(46.20±4.39)U/L、(53.09±17.30)U/L,健康对照组的分别为(25.70±6.45)mm、(7.5±2.5)个、(14.61±4.04)cm/s、(0.59±0.16)、(215.00±24.39)pmol/L、(7.10±1.57)U/L、(20.00±10.23)U/L,两组间的差异均有统计学意义(t=2.21、7.20、2.54、2.23、5.34、5.34、2.26、2.16,P均<0.05)。POF组PSV与E2水平呈正相关(r=0.443,P<0.05),MOD、AFC与E2水平无相关性,MOD、AFC、PSV均与FSH水平呈负相关(r=-0.440,P<0.05;r=-0.409,P<0.05;r=-0.556,P<0.01),RI与E2、FSH水平无相关性。结论 POF患者MOD、AFC、PSV均与FSH水平显著相关。 Objective To study the relationship between ovarian diameter,blood index parameters and sex hormone level in premature ovarian failure(POF)patients.Methods A total of 32 female patients who suffered POF were included in POF group,and 40 healthy females were included in control group.The maximum ovarian diameter(MOD),antral follicle count(AFC),peak systolic velocity(PSV),resistance index(RI)by transvaginal color Doppler ultrasonography estradiol(E2),basci follicle-stimulating hormone(FSH)and luteinizing hormone(LH)levels of two groups were analyzed retrospectively.Results The MOD,AFC,PSV,RI,E2,FSH,LH respectively were(18.10±4.93)mm,0,(6.12±2.60)cm/s,(0.67±0.32),(72.20±10.45)pmol/L,(46.20±4.39)IU/L,(53.09±17.30)IU/L,and the healthy control group were(25.70±6.45)mm,(7.5±2.5),(14.61±4.04)cm/s,(0.59±0.16),(215.00±24.39)pmol/L,(7.10±1.57)IU/L,(20.00±10.23)IU/L.The differences between the two groups were statistically significant(t=2.21,7.20,2.54,2.23,5.34,5.34,2.26,2.16,all P<0.05).In POF group,there was a positive correlation between PSV and E2(r=0.443,P<0.05).There was no relation in MOD,AFC and E2.The negative correlation existed between FSH and MOD or AFC or PSV(r=-0.440,P<0.05;r=-0.409,P<0.05;r=-0.556,P<0.01),There was no relation between RI and E2,FSH.Conclusion The MOD,AFC,PSV of POF patients are significantly correlated with FSH levels.
