免疫抑制剂对肾移植受者外周血单个核细胞Foxp3 mRNA表达的影响

Expression of Foxp3 mRNA in peripheral blood mononuclear cells of kidney transplant patients receiving different immunosuppressive protocols

摘要 目的探讨免疫抑制剂对肾移植受者外周血单个核细胞中叉状头螺旋转录因子Foxp3mRNA表达的影响。方法采用实时荧光定量PCR法检测钙调磷酸蛋白酶抑制剂(CNI)组、雷帕霉素组、终末期肾病组以及正常对照组新鲜外周血单个核细胞中Foxp3mRNA的表达水平,苦味酸法检测同期血肌酐浓度,比较各组间Foxp3mRNA表达水平和肌酐浓度的差异。结果CNI组(0.1089±0.0967)的外周血Foxp3mRNA表达水平低于雷帕霉素组(1.1264±0.6565)、正常对照组(0.9380±0.3614)和终末期肾病组(1.1200±0.5703),差异均有统计学意义(均P<0.01)。雷帕霉素组[(173.58±44.65)μmol/L]与CNI组[(150.45±46.21)μmol/L]血肌酐浓度差异无统计学意义(P>0.05)。结论CNI类免疫抑制剂抑制了肾移植后免疫耐受的诱导,而雷帕霉素可能参与了免疫耐受的诱导和维持。 Objective To investigate the relationship between the expression of forkhead helix transcription factor (Foxp3) mRNA and creatinine in peripheral blood mononuclear cells (PBMC) and different immunosuppressive protocols in kidney transplant recipients.Methods Real time quantitative PCR was used to examine Foxp3 mRNA expression in the PBMC from the subjects of calcineurin inhibitors group,rapamycin group,end-stage renal disease group,and normal control group.Their creatinine levels were simultaneously measured by a trinitrophenol method.Foxp3 mRNA and creatinine were compared between the groups.Results The Foxp3 mRNA expression was significantly lower in the calcineurin group (0.1089±0.0967) as compared to that in the rapamycin group (1.1264±0.6565),normal control group (0.9380±0.3614),and end-stage renal disease group (1.1200±0.5703),P<0.01.No significant difference was found in the level of creatinine between the rapamycin group [(173.58±44.65) μmol/L] and calcineurin group [(150.45±46.21) μmol/L,P>0.05].Conclusion CNI may suppress the induction of immune tolerance after kidney transplantation,while rapamycin may play a more important role in inducing and maintaining graft immune tolerance.
