受试者特征曲线评价脉冲振荡肺功能在支气管哮喘诊断中的应用价值 被引量:2

ROC curve estimates the value of impulse oscinometry to diagnosing bronchial asthma

摘要 目的应用受试者特征(ROC)曲线评价脉冲振荡(IOS)肺功能参数对各严重程度支气管哮喘(简称哮喘)的诊断价值。方法测定64例哮喘患者及23名健康人的常规肺功能(FEV1%pred、FEV1/FVC)及IOS参数(Z5、R5、Rc、Rp、R20、X5、Fres),并依据《支气管哮喘防治指南(支气管哮喘的定义、诊断、治疗和管理方案)》将哮喘患者按病情严重程度分级分为3组,其中轻度20例,中、重度各22例。各组间性别、年龄、身高、体质量差异无统计学意义(P>0.05)。采用ROC曲线对各哮喘亚组进行统计分析。结果①轻度哮喘组:ROC曲线下面积Z5=R5>Rc>Rp>R20>FEV1/FVC>X5>FEV1%pred>Fres,表明IOS肺功能测定对轻度哮喘的诊断效能明显优于常规肺功能;②中度哮喘组:ROC曲线下面积FEV1%pred>Z5=R5>FEV1/FVC>R20>Rc>Rp>Fres>X5,表明IOS肺功能测定对中度哮喘的诊断效能与常规肺功能相当,但IOS测定更为简便,并可提供较多的参考指标;③重度哮喘组:ROC曲线下面积FEV1%pred>Rp>X5>Z5>R5>Fres>FEV1/FVC>Rc>Rp,表明IOS肺功能测定对重度哮喘的诊断效能与常规肺功能相当,但重度哮喘患者往往不能配合常规肺功能测定,而IOS则容易被患者接受和完成测定。结论 IOS参数对轻度哮喘的诊断效能明显优于常规肺功能参数;对中度哮喘的诊断准确性亦较高,并且测定过程简便、快捷;对重度哮喘的诊断效能较高,并且容易被患者接受和完成测定。 Objective To estimate the value of impulse oscinometry in diagnosing of bronchial asthma(asthma).Methods That FEV1%pred,FEV1/FVC and parameter from IOS pulmonary function were measured in 64 asthma patients and 23 healthy controls.By severity,asthma patients were separated into three groups.ROC curve was used to analyze the diagnosing value of IOS in every asthma group.Results Mild asthma group:AUC:Z5=R5>Rc>Rp>R20>FEV1/FVC>X5>FEV1%pred>Fres;medium asthma group:AUC FEV1%pred>Z5=R5>FEV1/FVC>R20>Rc>Rp>Fres>X5;heavy asthma group:AUC:FEV1%pred>Rp>X5>Z5>R5>Fres>FEV1/FVC>Rc>Rp.Conclusions In the diagnosing of mild asthma,IOS is better than FEV1%pred and FEV1/FVC.In the diagnosing of medium or heavy asthma,IOS is more convenient.
