摘要
目的比较不同术式切除老年良性前列腺增生患者的前列腺增生组织在整个前列腺中的重量及比例。方法 2008年12月至2010年5月,对开放性耻骨上经膀胱前列腺摘除术、经尿道等离子前列腺剜除术(PKEP)和经尿道前列腺等离子双极电切术(PKRP)所切除的217例标本进行称重,与术前前列腺估计重量进行比较。结果 217例术前前列腺估计重量平均为59.1g。三组间差异无统计学意义(P>0.05)。开放手术组术后标本重量平均为22.4g,明显高于PKEP组和PKRP组的术后标本重量(P<0.01)。而PKEP组与PKRP组的术后标本重量差异无统计学意义(P>0.05)。开放手术摘除的前列腺增生组织重量占术前整个前列腺估计重量的38.6%,而PKEP和PKRP切除的标本重量分别占其术前估计重量的26.5%和25.4%。结论手术只能切除占整体前列腺38.6%的增生组织。而且对于熟练者,开放手术、PKEP和PKRP三种术式切除前列腺增生组织比例无明显差别。
Objective To explore the proportion of the hyperplastic tissue in the whole prostate of the patients with benign prostatic hyperplasia( BPH ). And to compare the weight of the resected tissue of different surgical operations of BPH. Methods From December 2008 to May 2010, Firstly, it was done by weighing the resected tissues of 217 patients with BPH undergone suprapubic prostatectomy, plasmakinetic enucleation of prostate( PKEP ) and plasmakinetic resection of prostate ( PKRP ). Then a comparison was conducted with the preoperative prostate estimated weight. Results The average preoperative estimated weight of the prostate of all patients was 59.1 g,and so there was no significantly statistical difference among the three groups of different sorts of surgical operations( P < 0.01 ). The average resected tissue weight of suprapubic prostatectomy group was 22.4 g,which was a little heavier than that of the PKEP group or the PKRP group ( P < 0.01 ), but there was no significant difference between the PKEP group and the PKRP group ( P > 0.05 ). Among the average preoperative estimated weight of the entire prostate, the average resected tissue weight after suprapubic prostatectomy accounted for 38.6 %,but that after PKEP was 26.5 %,and after PKRP was 25.4 % . Conclusions Surgical operation can only resect the hyperplastic tissue that is 36.8 % of the entire prostate. For a skilled operator, the proportion of the resected tissue in the three sorts of surgery shows no significant difference.
