颈动脉粥样硬化斑块的影像学比较与缺血性脑卒中的相关性研究

Study on the Diagnosis of Carotid atherosclerotic Plaque Imaging and Assess the Relationship between Cerebral Infarction and the Stability of the Carotid Atherosclerotic Plaque

目的通过对颈动脉粥样硬化斑块的超声、CT、MRI的影像对比观察,探讨不同性质斑块的影像学表现及其特征以及斑块的稳定性与脑梗死发生的关系。方法对30例脑梗死患者和30例健康对照者进行检查,应用彩色多普勒超声仪、CT、MRI检测颈动脉,分析斑块大小、形态、特点,评估其稳定性,经统计学分析,探讨斑块不同分型与脑梗死发生的相关性。结果 30例患者经彩色多普勒超声共检出不同类型粥样硬化斑块22例共57处,对照组中有4例发生不同程度颈动脉粥样硬化,按常规B超斑块分型,对脑梗死发生诊断比较,差异有统计学意义(P<0.05);CT检查共检出25处钙化性斑块,对照组中见4处点条状钙化;MRI共检查斑块23例53处,斑块以IV～V型为主,斑块表面形态特征与脑梗死相关性分析,差异有统计学意义(P<0.05)。结论无创伤性影像检查应用于颈动脉粥样硬化斑块的检测,对粥样硬化斑块的性质能做出准确的评估,为早期发现和预防心脑血管疾病提供重要的依据。

Objective To investigate the value of noninvasive imaging in the diagnostic of carotid atherosclerotic plaque and to assess the relationship between cerebral infarction and the stability of the carotid atherosclerotic plaque.Methods 30 patients in patients with cerebral infarction and 30 cases of healthy controls were for inspection,the application of color doppler ultrasound,CT and MRI testing instrument carotid artery and analysis patch size,shape,characteristics,and evaluate the stability,the statistical analysis,this paper discusses the different classification and plaques cerebral infarction occurred relevance.Results 30 patients by color doppler ultrasound detection of different types of atherosclerotic plaque 22 cases of 57 place,in the control group,there were 4 cases different degree carotid atherosclerosis,according to the conventional B super plaques points type,for cerebral infarction occurred was statistically significant(P<0.05);CT examination there were 25 calcified plaque,in the control group,see 4 point shape calcification;A total of 53 MRI inspection plaque,The plaque is dominated by IV～V type,plaque surface morphology of cerebral infarction and correlation analysis statistical significance.Conclusion Noninvasive imaging applied in the detection of carotid atherosclerosis plaque,plaque nature can make accurate assessment,for the early detection and prevention of cardiovascular and cerebrovascular diseases provide important basis..
