胆脂瘤型中耳炎手术方式的临床探讨

Clinical Surgical Efficacy Analysis of Cholesteatoma Otitis Media

目的:探讨胆脂瘤型中耳炎的手术方式及临床效果。方法:回顾性分析本院耳鼻咽喉科2008年2月-2011年10月收治的68例胆脂瘤型中耳炎患者,行一期乳突鼓室成形术治疗,比较切除外耳道后壁的乳突鼓室成形术(CWD32例)与保留外耳道后壁的乳突鼓室成形术(CWU36例)两种常用术式的临床效果。结果:术后随访6个月,CWU组干耳时间明显短于CWD组;CWU组语频区听力提高幅度及气骨导差缩小程度明显优于CWD组,两组比较差异显著有统计学意义(P<0.05)。结论:胆脂瘤型中耳炎的行乳突鼓室成形术治疗,最大限度地保留或恢复听力功能为本病外科治疗的重要目标之一。保留外耳道后壁的乳突鼓室成形术,病变清除彻底同期听力重建术后疗效满意,需进一步临床观察。

Objective:To investigate the surgical efficacy of cholesteatoma otitis media.Methods:The data of 68 cases of first Stage tympallomastoidectomy cholesteatoma otitis media,were analyzed retrospectively,from Feb 2008 to Oct 2011,wich discuss on the procedures cdanal wall up and cdanal wall downtympallomastoidectomy.Results:All cases wre followed up for 6 months.Duration of getting dry ear,CWD group was longer than CWU group(P < 0.05).on the contrast of inmproved hearing level,CWU group was batter than CWD group(P < 0.05).Conclusion:To treat the choloresteatomatis media by tympanoplasty in ordeir to reserve or restore hearing as much as possible has become an inlportant goal.cdanal wall up diseased region eompletdy and reconstructing the hearing,but further clinical observatlon is repuired.
