腹腔镜联合胃镜治疗胃间质瘤23例体会

Treatment of gastric stromal tumor through laparoscopy combined with gastroscopy

目的总结腹腔镜联合胃镜行胃间质瘤切除的经验。方法对2010年2月至2012年2月间我院收治的23例胃间质瘤(直径<5cm)的患者应用腹腔镜联合胃镜术中定位技术予以手术切除治疗及总结治疗的效果。结果 23例胃间质瘤均在腹腔镜联合胃镜下顺利找到肿瘤并成功予以局部切除,无中转开腹病例。手术时间为45min～110min,平均(65.9±7.3)min;术中出血量为5mL～35mL,平均(13.9±8.4)mL;术后住院天数4d～8d,平均(6.1±1.4)d。结论应用腹腔镜联合胃镜治疗胃间质瘤具有术中定位准确、手术安全、手术时间短、创伤小、恢复快等优点。

Objective To summarize the experience of laparoscopy combined with gastroscopy for treating gastrointestinal stromal tumors(GSTs).Methods From Feb.2010 to Feb.2012,twenty-three patients with GST received gastric tumorectomy by using laparoscopic and gastroscopic procedure according to the intraoperative location of tumor in laparoscopy combined with gastroscopy.Results All of 23 cases were successfully performed partial gastrectomy under laparoscopy combined with gastroscopy.No cases were transferred to open laparotomy.The operation time ranged from 45 minutes to 110 minutes,an average of(65.9±7.3) minutes.Blood loss during operation procedure was 5 mL to 35 mL,an average of(13.9±8.4)mL.Postoperative hospital stay was in 4 days to 8 days,an average of(6.1 ±1.4) days.Conclusion Treatment of GSTs under laparoscopy combined gastroscopy has the advantages of accurate intraoperative positioning,surgical safety,shorter operative time,less trauma,quick recovery.
