正畸治疗中的牙弓整体移动 被引量:2

摘要 早在20世纪70年代,Merrifield就提出以牙齿、咬合、骨骼等多方综合因素为中心的正畸诊断、设计和治疗的方案.因此,错[牙合]畸形的概念已经从单纯的牙齿不齐转变为牙 合、颌、面的不协调.对于现代的正畸医师而言,高效的固定矫正器很容易将牙齿排列整齐,但是排齐牙齿并不一定会获得良好的咬合功能和美观的容貌.因此,解决上下牙弓之间的三维关系,同时兼顾上下颌骨之间的关系显得尤为重要.正畸医师在青春期和成年患者中使用了很多的方法来改善骨骼的生长,比如肌激动器、前方牵引器、颏兜、Frankle矫正器等.这些矫正器也有一定的矫形作用.但是通过正畸治疗来改善上下颌骨位置的结果往往没有那么乐观[1-2].正畸治疗对于骨骼畸形的治疗并没有十足的把握,正畸医师最擅长的是熟练运用各种技能进行牙齿的移动.事实上正畸治疗中上下牙弓之间关系的调整在很大程度上是牙齿的移动,而骨骼移动只占很小一部分甚至没有(或者在治疗后复发).因此,牙弓的移动是正畸治疗中非常重要的部分.在本文中,我们将探讨正畸治疗中牙弓整体移动的可行性和方法,探讨牙弓整体移动与调整咬合和颌骨间的关系.
