纳米碳引导下腹腔镜中低位直肠癌根治术的近期疗效分析

目的探讨纳米碳引导下腹腔镜中低位直肠癌根治术的近期疗效。方法 2007年5月至2011年1月共有73例中低位直肠癌患者在兰州军区乌鲁木齐总医院普外科接受手术治疗,按照手术方式分为腔镜组(35例)和开腹组(38例),腔镜组在纳米碳引导下行腹腔镜中低位直肠癌根治术,开腹组采用传统的开腹手术;回顾性比较分析两组患者手术及术后恢复情况。结果腔镜组手术平均时间明显长于开腹组,但术中出血量明显少于开腹组,差异均具有统计学意义(P<0.05);两组患者在淋巴结清扫数目、肿块距下缘距离及远端切缘阳性数等方面相差不明显,差异不具有统计学意义(P>0.05)。术后,腹腔镜组患者胃肠道恢复时间明显短于开腹组,差异具有统计学意义(P<0.05),但两组患者术后并发症的发生率差异不具有统计学意义(P>0.05)。结论纳米碳引导下腹腔镜中下段直肠癌根治术具有安全、可靠及手术创伤小的优点,近期疗效满意,值得进一步进行摸索与研究。

Objective To investigate the curative effect of mid-low rectal cancer in the laparoscopic which guided by carbon nanoparticles.Methods May 2007 to January 2011,73 cases patients of mid-low rectal cancer underwent surgery in department of general surgery of the Urumqi General Hospital of Lanzhou Military Region,all patients were divided into laparoscopic group(35 cases)and laparotomy group(38 cases)according to the surgical approach.The patients of laparoscopic group were accepted the laparoscopic which guided by carbon nanoparticles,and the patients of laparotomy group were accepted the laparotomy.The outcomes of two patient groups with radical surgery were prospectively evaluated.Results The mean operative time of the laparoscopic group was significantly longer than the laparotomy group,but the intraoperative blood loss was significantly less than the laparotomy group,the differences were statistically significant(P<0.05).The two groups in the number of lymph node dissection,the distance of tumor from the lower edge and the positive number of distal margin difference have not obvious different,the difference was not statistically significant(P>0.05);Postoperative,the laparoscopic group of patients with gastrointestinal recovery time is significantly shorter than in the laparotomy group,the differences were statistically significant(P<0.05),but the difference in the incidence of postoperative complications in two groups of patients is not statistically significant(P>0.05).Conclusions The mid-low rectal cancer in the laparoscopic which guided by carbon nanoparticles,have the advantages of safe,reliable and minimally invasive,with similar radical resection and satisfactory short-term outcomes,worthy of further exploration and research.
