耐高浓度氨基糖苷类抗生素的肠球菌检测与分析

摘要 目的 :检测临床标本中耐高浓度氨基糖苷类抗生素的肠球菌与耐万古霉素肠球菌分布及耐药频率 ,以配合院内感染监控工作开展 ,指导临床合理用药。方法 :依据全国临床检验操作规程进行肠球菌分离及药敏试验。结果 :从临床标本中分离 4 6 6株病原菌 ,其中肠球菌 6 3株占 13 5 %。肠球菌对庆大霉素高耐药率 6 9 8% ,低耐药率 6 3% ,敏感率 2 3 8% ;对链霉素高耐药率 74 6 % ,低耐药率 2 0 6 % ,敏感率4 8%。肠球菌高耐率分别占庆大霉素与链霉素耐药率 91 7%与 78 3%。尚未发现万古霉素耐药株。结论 :肠球菌对氨基糖苷类抗生素的高度耐药性应引起临床和检验工作者的高度重视。 Objective:To investigate the distribution and the frequency of the high-level aminoglycosices and vancomycin resistance in clinical Enterococcus isolates, and to aid in the supervision of nosocomial infections, rational use of antibiotics clinically. Methods: Isolation and antibiotic susceptibility test of Enterococcus were performed according to the National Clinical Laboratory Standards. Results: Of the 466 clinical isolates, Enterococcus was 63 strains (13.5%).The susceptibility of Enterococus to gentamycin was 23.8%, 6.3% of them belonged to low-level resistant and 69.8% high-level resistant. The susceptibility of Enterococci to Streptomycin was 4.8%, 20.6% of them was low-level resistant and 74.6% high-level resistant. The high-level aminoglycosices and streptomycin resistant strains of Enterococcus were 91.7% and 78.3%, respectively. No vancomycin resistant strain was found. Conclusion: The high-level resistance of Enterococcus to the aminoglycosices should be greatly watched by the clinician and the laboratory technician.
