居家宁养的晚期癌症病人生存现况调查及护理干预

The survival inventory survey and nursing intervention in advanced cancer patients of home hospice care

目的调查居家宁养晚期癌症病人生存现状,通过护理干预,使癌症病人生存现状得以改善。方法自行设计调查问卷,向癌症病人直系亲属调查其生存现状,将入选的80例居家宁养的癌症病人随机分为对照组与试验组各40例,对照组采取病人或家属随机向病区2名责任护士组长电话咨询护理问题的方式,护理方式为被动服务;试验组采取病区2名责任护士组长向病人或家属电话随访的护理方式,每周1次,必要时上门服务,护理方式为主动服务;对2组癌症病人在压疮、呼吸困难、癌痛、心理障碍、恶心呕吐、腹胀等方面进行统计分析。结果 2组病人除腹胀外,在压疮、呼吸困难、癌痛、心理障碍、恶心呕吐等方面差异均有统计学意义(P均<0.05)。结论护理服务的延伸有助于癌症病人生存现状的改善,护理服务理念的升华带来经济效益和社会效益的双赢。

Objective To investigate the survival situation of patients with advanced cancer through the nursing intervention. Methods Survival situation of cancer patients was investigated through their directly-related members by questionnaire survey. Eighty cancer patients of home hospice care were randomly divided into control group and experiment group.In control group,the patients or family members related with two duty nurse leaders counseling nursing questions; In experiment group, two duty nurse leaders related with the patients or families by telephone once a week,and necessary door-to-door service.The pressure sores, difficulty breathing, pain, the psychological barrier, nausea vomiting, abdominal distension were analyzed between the two groups. Results In addition to abdominal distension, there were significant differences between two groups in pressure sores, difficulty breathing, pain, the psychological barrier, nausea and vomiting (P<0.05). Conclusions Extending of nursing services can help to improve the living situation of patients with cancer, and nursing service concept can bring economic benefit and social benefit.
